14 On page 13, line 21, through page 14, line 6, delete
15 those lines

17 and insert: such termination. However, if termination was the
18 result of failure to pay any required premium or contribution
19 and such nonpayment of premium was due to acts of an employer
20 or group contract holder other than the employee or individual
21 subscriber, written application for the contract must be made
22 and the first premium must be paid not later than 63 days
23 after notice of termination is mailed by the organization or
24 the employer, whichever is earlier, to the employee's or
25 individual's last address as shown by the record of the
26 organization or the employer, whichever is applicable. In such
27 case of termination due to non-payment of premium by the
28 employer or group contract holder, the premium for the
29 converted contract may not exceed the rate for the prior group
30 coverage for the period of coverage under the converted
31 contract prior to the date notice of termination is mailed to
