The notary public must legibly note on the 22 consent the date and time the consent was executed. The 23 witnesses' names must be typed or printed underneath their 24 signatures, and their home or business addresses, and social 25 security numbers, or drivers' license numbers must be 26 included. The person signing the consent has the right to have 27 at least one of the witnesses be an individual who does not 28 have a partnership, employment, agency, or other professional 29 or personal relationship with the adoption entity or the 30 prospective adoptive parents.
