14         On page 5, between lines 3 and 4 of the bill

16  insert:

17         6.  Notice is waived if the referring physician or

18  physician performing the abortion documents in the patient's

19  medical records that the minor is sufficiently mature to

20  decide whether to terminate her pregnancy, the physician has

21  advised the minor to talk to her parent or parents, and that

22  the minor has receive counsel from an adult relative or

23  licensed physician, psychologist, social worker, nurse

24  practitioner, guidance counselor, practical nurse, registered

25  nurse, or ordained member of the clergy.
