14 On page 4, between lines 19 and 20, of the bill

16 insert:
17 (b) A vasectomy may not be performed unless the person
18 performing the vasectomy has given actual notice to the wife
19 of the patient at least 48 hours prior to performing the
20 vasectomy. However, a patient seeking a vasectomy may
21 petition any circuit court for a waiver of the notice
22 requirement. The petition shall include a statement that the
23 patient has the ability to produce offspring, that he wishes
24 to terminate that ability, and that notice to his wife could
25 cause him serious physical or emotional harm.
