13         Senate Amendment (with title amendment) 

14         On page 17, between lines 6 and 7,

16  insert:

17         Section 7.  Subsection (1) of section 627.6408, Florida

18  Statutes, is amended to read:

19         627.6408  Diabetes treatment services.--

20         (1)  A health insurance policy or group health

21  insurance policy sold in this state must provide coverage for

22  all medically appropriate and necessary equipment, supplies,

23  and diabetes outpatient self-management training and

24  educational services used to treat diabetes, if the patient's

25  treating physician or a physician who specializes in the

26  treatment of diabetes certifies that such services are

27  necessary. Medications approved by the United States Food and

28  Drug Administration which are certified as necessary by the

29  patient's treating physician or a physician who specializes in

30  the treatment of diabetes shall be covered as provided by

31  contract.

 1         Section 8.  Subsection (1) of section 627.65745,

 2  Florida Statutes, is amended to read:

 3         627.65745  Diabetes treatment services.--

 4         (1)  A health insurance policy or group health

 5  insurance policy sold in this state, including a health

 6  benefit plan issued pursuant to s. 627.6699, must provide

 7  coverage for all medically appropriate and necessary

 8  equipment, supplies, and diabetes outpatient self-management

 9  training and educational services used to treat diabetes, if

10  the patient's treating physician or a physician who

11  specializes in the treatment of diabetes certifies that such

12  services are necessary. Medications approved by the United

13  States Food and Drug Administration which are certified as

14  necessary by the patient's treating physician or a physician

15  who specializes in the treatment of diabetes shall be covered

16  as provided by contract.
