14         On page 7, lines 25-31,

15  remove:  all of said lines

17  and insert:  licensed physician or licensed dentist who

18  provides the medical service benefits or procedures which are

19  within the scope of a licensed physician's license or licensed

20  dentist's license. Any limitation or condition placed upon

21  payment to, or upon services, diagnosis, or treatment by, any

22  licensed physician shall or licensed dentist apply equally to

23  all licensed physicians without unfair discrimination to the

24  usual and customary treatment procedures of any class of

25  physicians or licensed dentist.
