| 11 | Be It Enacted by the Legislature of the State of Florida: |
| 12 |
|
| 13 | Section 1. Subsection (6) of section 322.08, Florida |
| 14 | Statutes, is amended to read: |
| 15 | 322.08 Application for license.-- |
| 16 | (6) The application form for a driver's license or |
| 17 | duplicate thereof shall include language permitting the |
| 18 | following: |
| 19 | (a) A voluntary contribution of $5 per applicant, which |
| 20 | contribution shall be transferred into the Election Campaign |
| 21 | Financing Trust Fund. |
| 22 | (b) A voluntary contribution of $1 per applicant, which |
| 23 | contribution shall be deposited into the Florida Organ and |
| 24 | Tissue Donor Education and Procurement Trust Fund for organ and |
| 25 | tissue donor education and for maintaining the organ and tissue |
| 26 | donor registry. |
| 27 | (c) A voluntary contribution of $1 per applicant, which |
| 28 | contribution shall be distributed to the Florida Council of the |
| 29 | Blind. |
| 30 | (d) A voluntary contribution of $2 per applicant, which |
| 31 | shall be distributed to the Hearing Research Institute, |
| 32 | Incorporated, for the purpose of infant hearing screening in |
| 33 | Florida. |
| 34 | (e) A voluntary contribution of $1 per applicant, which |
| 35 | shall be distributed to the Juvenile Diabetes Foundation |
| 36 | International. |
| 37 | (f) A voluntary contribution of $2 per applicant, which |
| 38 | shall be distributed to the Florida Prostate Cancer Network, |
| 39 | notwithstanding s. 322.081. |

| 41 | A statement providing an explanation of the purpose of the trust |
| 42 | funds shall also be included. |
| 43 | Section 2. This act shall take effect July 1, 2004. |
