14         On page 2, between lines 28-29,
15  
16  insert:  
17         Section 2.  Use of social security number on
18  identification cards.--An entity that contracts with the state
19  to provide health insurance coverage for state employees shall
20  not issue an identification card for use by a covered employee
21  which:
22         (1)  Contains that employee's social security number,
23  or
24         (2)  Contains that employee's social security number as
25  part of another identifier, either by adding additional
26  prefixes, suffixes, numbers or letters.

 3         Delete everything before the enacting clause
 4  
 5  and insert:
 6                  A bill to be entitled
 7         An act relating to the use of social security
 8         numbers; creating s. 1004.09, F.S.; prohibiting
 9         certain uses of student social security numbers
10         by state universities, community colleges, and
11         public postsecondary technical centers;
12         requiring each state university, community
13         college, and public postsecondary technical
14         center to review policies and procedures and
15         submit a plan; requiring each state university,
16         community college, and public postsecondary
17         technical center to annually document
18         compliance in a report; prohibiting the use of
19         social security numbers as identification
20         numbers on health insurance cards used by state
21         employees; providing for injunctive relief;
22         providing an effective date.
