| 5 | b. The manufacturer of the prescription drug shipped |
| 6 | directly to the recipient must provide and the recipient of the |
| 7 | prescription drug must acquire, within 14 days after receipt of |
| 8 | the prescription drug, a shipping document from the manufacturer |
| 9 | that contains, at a minimum: |
| 10 | (I) The name and address of the manufacturer, including |
| 11 | the point of origin of the shipment; the wholesaler; and such |
| 12 | purchaser. |
| 13 | (II) The name of the prescription drug as it appears on |
| 14 | the label. |
| 15 | (III) The quantity, dosage form, and strength of the |
| 16 | prescription drug. |
| 17 | (IV) The date of the shipment from the manufacturer. |

| 19 | The wholesale distributor must also maintain and make available |
| 20 | to the department, upon request, the lot number of the |
| 21 | prescription drug if the lot number is not contained in the |
| 22 | shipping document acquired by the recipient. |
