11  Senator Saunders moved the following amendment:

13         Senate Amendment (with title amendment) 
14         On page 23, lines 9-29, delete those lines
15  
16  and insert:  
17         Section 11.  Present subsection (4) of section 744.341,
18  Florida Statutes, is redesignated as subsection (5), and a new
19  subsection (4) is added to that section, to read:
20         744.341  Voluntary guardianship.--
21         (4)  A guardian must include in the annual report filed
22  with the court a certificate from a licensed physician who
23  examined the ward not more than 90 days before the annual
24  report is filed with the court. The certificate must certify
25  that the ward is competent to understand the nature of the
26  guardianship and of the ward's authority to delegate powers to
27  the voluntary guardian.
28         (5)(4)  A voluntary guardianship may be terminated by
29  the ward by filing a notice with the court that the voluntary
30  guardianship is terminated. A copy of the notice must be
31  served on all interested persons.
