| 8 | prohibited; penalties; exceptions.-- |

| 10 | (e) The division shall maintain accurate records and |
| 11 | statistics regarding injuries incurred by greyhounds that race |
| 12 | in this state. The division shall adopt rules requiring the |
| 13 | reporting of injuries incurred by greyhounds while racing in |
| 14 | this state, including schooling races. Such reports must |
| 15 | include: |
| 16 | 1. The greyhound's registered name and right and left ear |
| 17 | tattoo numbers. |
| 18 | 2. The name, business address, and telephone number of the |
| 19 | greyhound owner, trainer, and kennel operator. |
| 20 | 3. The color, weight, and sex of the greyhound. |
| 21 | 4. The specific type of injury, the cause of the injury, |
| 22 | the estimated recovery time, and the location of the injury on |
| 23 | the greyhound. |
| 24 | 5. Where the injury occurred, whether on a racing track or |
| 25 | in another area. |
| 26 | 6. If the injury occurred while the greyhound was racing, |
| 27 | the racetrack where the injury occurred; the distance, grade, |
| 28 | race, and post position when the injury occurred; and the |
| 29 | weather conditions, time, temperature, and track condition at |
| 30 | the time of the injury. |
| 31 | 7. A certification by the racetrack veterinarian that the |
| 32 | form is correct. |
| 33 | (f) The division shall maintain accurate records and |
| 34 | statistics regarding the disposition of greyhounds that |
| 35 | participate in racing in this state. The division shall adopt |
| 36 | rules requiring the reporting of the disposition of greyhounds |
| 37 | that race in this state, including schooling races. As used in |
| 38 | the reporting requirement, the term "disposition" means death, |
| 39 | transfer to another jurisdiction, retirement, adoption, sale, or |
| 40 | donation for medical research or another purpose. Such reports |
| 41 | must include: |
| 42 | 1. The greyhound's registered name and right and left ear |
| 43 | tattoo numbers; the name, business address, and telephone number |
| 44 | of the greyhound owner, trainer, and kennel operator; and the |
| 45 | name and address of the race track where the greyhound last |
| 46 | raced prior to disposition. |
| 47 | 2. If the greyhound was transferred to another track, the |
| 48 | name and address of the track that received the greyhound and |
| 49 | the name, business address, telephone number, and driver's |
| 50 | license number and state of issuance of the person who received |
| 51 | the greyhound on behalf of that track. |
| 52 | 3. If the greyhound was retired for breeding, the name and |
| 53 | address of the facility that received the greyhound and the |
| 54 | name, business address, telephone number, and driver's license |
| 55 | number and state of issuance of the person who received the |
| 56 | greyhound on behalf of that facility. |
| 57 | 4. If the greyhound was adopted or placed for adoption, |
| 58 | the name and address of the person that received the greyhound |
| 59 | and, if applicable, the name, business address, telephone |
| 60 | number, and driver's license number and state of issuance of the |
| 61 | person who received the greyhound on behalf of the adoption |
| 62 | facility. |
| 63 | 5. If the greyhound was euthanized, the name, address, |
| 64 | professional title, professional affiliation of the person |
| 65 | performing the euthanasia, method of euthanasia, and reason the |
| 66 | greyhound was euthanized rather than adopted. |
| 67 | 6. If the greyhound was sold or donated, the name of the |
| 68 | person to whom the greyhound was sold or donated, and if |
| 69 | donated, the name, business address, telephone number, and |
| 70 | driver's license number and state of issuance of the person who |
| 71 | received the greyhound on behalf of the donee. |
| 72 | 7. If the disposition of the greyhound does not fit into |
| 73 | any of the above categories, the name of the person to whom the |
| 74 | greyhound was transferred, and the name, business address, |
| 75 | telephone number, and driver's license number and state of |
| 76 | issuance of the person who received the greyhound. |
| 77 | 8. Certification by the owner, trainer, and kennel |
| 78 | operator that the disposition forms are correct. |
| 79 | (g) The division shall maintain injury and disposition |
| 80 | records for 7 years. |
| 81 | (h) In addition to other penalties imposed by law, a |
| 82 | person who knowingly makes a false statement on an injury or |
| 83 | disposition form commits a misdemeanor of the first degree, |
| 84 | punishable as provided in s. 775.082 or s. 775.083. A person who |
| 85 | knowingly makes a false statement on an injury or disposition |
| 86 | form on a second or subsequent occasion commits a felony of the |
| 87 | third degree, punishable as provided in s. 775.082, s. 775.083, |
| 88 | or s. 775.084. |
