     (6) An individual elector's signature on a petition form

8

may be revoked only if such elector submits to the appropriate

9

supervisor of elections a signed petition-revocation form adopted

10

by rule for such purpose by the division and obtained by a

11

written request directly initiated by the individual elector. The

12

petition-revocation form for each corresponding petition form

13

under this code must be developed by the division and approved by

14

the Secretary of State and made available to any elector

15

requesting the form in writing. A valid petition-revocation form

16

must contain the original signature of the individual elector,

17

and the individual elector must accurately record on the form

18

provided to the individual elector directly by the appropriate

19

supervisor of elections the date on which he or she signed the

20

petition-revocation form, as well as the individual elector's

21

name, street address, county, and voter registration number or

22

date of birth. An individual elector's petition-revocation form

23

shall be submitted by the individual elector to the appropriate

24

supervisor of elections no later than 90 days after the date of

25

the individual elector's original petition signature. The

26

supervisor of elections receiving a petition-revocation form

27

directly from an individual elector shall promptly verify the

28

signature on the petition-revocation form and process such

29

revocation within 30 days after the date on which the form is

30

submitted. Any petition-revocation form verified after the

31

February 1 immediately preceding the next general election shall

32

not be considered valid unless the applicable initiative is not

33

certified for ballot position in that election. This section does

34

not authorize a third party to solicit any individual's petition

35

revocation or the creation of any committee to solicit the

36

revocation of any individual's petition signature.

     (a) An elector's signature on a petition form may be

38

revoked within 150 days of the date on which he or she signed the

39

petition form by submitting to the appropriate supervisor of

40

elections a signed petition-revocation form adopted by rule for

41

this purpose by the division.

42

     (b) The petition-revocation form and the manner in which

43

signatures are obtained, submitted, and verified shall be subject

44

to the same relevant requirements and timeframes as the

45

corresponding petition form and processes under this code and

46

shall be approved by the Secretary of State before any signature

47

on a petition-revocation form is obtained.

48

     (c) Supervisors of elections shall provide petition-

49

revocation forms to the public at all main and branch offices.

50

     (d) The petition-revocation form shall be filed with the

51

supervisor of elections by February 1 preceding the next general

52

election or, if the initiative amendment is not certified for

53

ballot position in that election, by February 1 preceding the

54

next successive general election. The supervisor of elections

55

shall promptly verify the signature on the petition-revocation

56

form and process such revocation upon payment, in advance, of a

57

fee of 10 cents or the actual cost of verifying such signature,

58

whichever is less. The supervisor shall promptly record each

59

valid and verified petition-revocation form in the statewide

60

voter registration system in the manner prescribed by the

61

Secretary of State.
