       The Committee on Judiciary (Joyner) recommended the following:
       
    1         Senate Amendment 
    2  
    3         Delete lines 264 - 267
    4  and insert:
    5  care of the principal and who is licensed to practice medicine
    6  or osteopathic medicine pursuant to chapter 458 or chapter 459
    7  as of the date of the affidavit. The affidavit executed by the
    8  physician must state that the physician is licensed to practice
    9  medicine or osteopathic medicine pursuant to chapter 458 or
   10  chapter 459, that the physician is the primary
