    4  and insert:
    5         (3) Any owner or lessee of a motor vehicle may request to
    6  participate in the program in the manner prescribed by the
    7  governing body of the county. A participant shall receive a
    8  yellow dot decal, a yellow dot folder, and a form on which the
    9  participant shall provide his or her personal and medical
   10  information.
   11         (a) The form must include a statement that the information
   12  provided will be disclosed only to authorized personnel of law
   13  enforcement and public safety agencies, emergency medical
   14  services agencies, and hospitals for the purposes authorized in
   15  subsection (5).
   16         (b) The form must describe the confidential nature of the
   17  medical information voluntarily provided by the participant and
   18  must include a notice to the participant stating that, by
   19  providing the medical information and signing the form, he or
   20  she agrees to the disclosure of the medical information to
   21  authorized personnel and their use of such information solely
   22  for the purposes listed in subsection (5).
   23         (c) The county may not charge a fee to participate in the
   24  yellow dot program.
   25         (4)(a) The participant shall affix the decal onto the rear
