The Department of Juvenile Justice must also provide a report of serious incidents to the Governor, the President of the Senate, and the Speaker of the House of Representatives on no less than a quarterly basis. The report must include, at a minimum: the number of incidents and allegations of staff abuse or abuse by another child, including whether or not an allegation was substantiated; descriptions of incidents or allegations of such abuse that resulted in physical injury or significant psychological trauma, or that involved deprivation of food, water, or medical care;995020 Log:0026 SC1/RMS 03/25/14 08:23:11 PM Senate Page: 1 and the failure of a provider to report incidents or allegations within required timeframes established by the department. The report must be organized so that the incidents and allegations relating to a particular facility and to a particular provider can be readily ascertained.
