       The Committee on Appropriations (Garcia) recommended the
       following:
       
    1         Senate Amendment (with title amendment)
    2  
    3         Delete lines 64 - 79
    4  and insert:
    5         1. Services may be provided in the hospital by the facility
    6  as well as by other health care practitioners who may separately
    7  bill the patient;
    8         2. Health care practitioners who provide services in the
    9  hospital may or may not participate with the same health
   10  insurers or health maintenance organizations as the hospital;
   11  and
   12         3. Prospective patients should contact the health care
   13  practitioner who will provide services in the hospital to
   14  determine which health insurers and health maintenance
   15  organizations the practitioner participates in as a network
   16  provider or preferred provider.
   17         (c) As applicable, the names, mailing addresses, and
   18  telephone numbers of the health care practitioners and medical
   19  practice groups with which it contracts to provide services in
   20  the hospital, and instructions on how to contact the
   21  practitioners and groups to determine which health insurers and
   22  health maintenance organizations they participate in as network
   23  providers or preferred providers.

   27         Delete lines 8 - 12
   28  and insert:
   29         post on its website certain information regarding
   30         health insurers, health maintenance organizations,
   31         health care practitioners, and practice groups that it
   32         contracts with, and a specified disclosure statement;
   33         amending s. 408.7057, F.S.;
