       The Committee on Health Policy (Berman) recommended the
       following:
       
    1         Senate Amendment (with title amendment)
    2  
    3         Delete lines 127 - 135
    4  and insert:
    5  consent; and
    6         4. Her age.
    7         (b) The department shall adopt by rule a form to be used
    8  for such monthly reports. Patient names may not be included on
    9  the forms.
