1 A bill to be entitled
2 An act relating to applications for driver licenses
3 and identification cards; amending s. 322.08, F.S.;
4 requiring application forms for original, renewal, and
5 replacement driver licenses and identification cards
6 to include language allowing a voluntary contribution
7 to the Armory Board for a specified purpose; exempting
8 such contributions from the General Revenue Fund
9 service charge; providing an effective date.

11 Be It Enacted by the Legislature of the State of Florida:

13 Section 1. Subsection (8) of section 322.08, Florida
14 Statutes, is amended to read:
15 322.08 Application for license; requirements for license
16 and identification card forms.—
17 (8) The application form for an original, renewal, or
18 replacement driver license or identification card must include
19 language permitting the following:
20 (a) A voluntary contribution of $1 per applicant, which
21 contribution shall be deposited into the Health Care Trust Fund
22 for organ and tissue donor education and for maintaining the
23 organ and tissue donor registry.
24 (b) A voluntary contribution of $1 per applicant, which
25 shall be distributed to the Florida Council of the Blind.
26 (c) A voluntary contribution of $2 per applicant, which
27 shall be distributed to the Hearing Research Institute,
28 Incorporated.
29 (d) A voluntary contribution of $1 per applicant, which
30 shall be distributed to the Juvenile Diabetes Foundation
31 International.
32 (e) A voluntary contribution of $1 per applicant, which
33 shall be distributed to the Children’s Hearing Help Fund.
34 (f) A voluntary contribution of $1 per applicant, which
35 shall be distributed to Family First, a nonprofit organization.
36 (g) A voluntary contribution of $1 per applicant to Stop
37 Heart Disease, which shall be distributed to the Florida Heart
38 Research Institute, a nonprofit organization.
39 (h) A voluntary contribution of $1 per applicant to Senior
40 Vision Services, which shall be distributed to the Florida
41 Association of Agencies Serving the Blind, Inc., a not-for
42 profit organization.
43 (i) A voluntary contribution of $1 per applicant for
44 services for persons with developmental disabilities, which
45 shall be distributed to The Arc of Florida.
46 (j) A voluntary contribution of $1 to the Ronald McDonald
47 House, which shall be distributed each month to Ronald McDonald
48 House Charities of Tampa Bay, Inc.
49 (k) Notwithstanding s. 322.081, a voluntary contribution of
50 $1 per applicant, which shall be distributed to the League
51 Against Cancer/La Liga Contra el Cancer, a not-for-profit
52 organization.
53 (l) A voluntary contribution of $1 per applicant to Prevent
54 Child Sexual Abuse, which shall be distributed to Lauren’s Kids,
55 Inc., a nonprofit organization.
56 (m) A voluntary contribution of $1 per applicant, which
57 shall be distributed to Prevent Blindness Florida, a not-for
58 profit organization, to prevent blindness and preserve the sight
59 of the residents of this state.
60 (n) Notwithstanding s. 322.081, a voluntary contribution of
61 $1 per applicant to the state homes for veterans, to be
62 distributed on a quarterly basis by the department to the
63 Operations and Maintenance Trust Fund within the Department of
64 Veterans’ Affairs.
65 (o) A voluntary contribution of $1 per applicant to the
66 Disabled American Veterans, Department of Florida, which shall
67 be distributed quarterly to Disabled American Veterans,
68 Department of Florida, a nonprofit organization.
69 (p) A voluntary contribution of $1 per applicant for Autism
70 Services and Supports, which shall be distributed to Achievement
71 and Rehabilitation Centers, Inc., Autism Services Fund.
72 (q) A voluntary contribution of $1 per applicant to Support
73 Our Troops, which shall be distributed to Support Our Troops,
74 Inc., a Florida not-for-profit organization.
75 (r) Notwithstanding s. 322.081, a voluntary contribution of
76 $1 per applicant to aid the homeless. Contributions made
77 pursuant to this paragraph shall be deposited into the Grants
78 and Donations Trust Fund of the Department of Children and
79 Families and used by the State Office on Homelessness to
80 supplement grants made under s. 420.622(4) and (5), provide
81 information to the public about homelessness in the state, and
82 provide literature for homeless persons seeking assistance.
83 (s) A voluntary contribution of $1 or more per applicant to
84 End Breast Cancer, which shall be distributed to the Florida
85 Breast Cancer Foundation.
86 (t) Notwithstanding s. 322.081(1), a voluntary contribution
87 of $1 or more per applicant to Childhood Cancer Care, which
88 shall be distributed to the Live Like Bella Childhood Cancer
89 Foundation.
90 (u) A voluntary contribution of $1 or more per applicant,
91 which shall be distributed to the Armory Board created under s.
92 250.40, to be used to maintain, repair, or renovate armories
93 within this state used for military purposes.

95 A statement providing an explanation of the purpose of the trust
96 funds shall also be included. For the purpose of applying the
97 service charge provided under s. 215.20, contributions received
98 under paragraphs (b)-(u) (b)-(t) are not income of a revenue
99 nature.
100 Section 2. This act shall take effect July 1, 2022.
