       Senator Osgood moved the following:
       
    1         Senate Amendment (with title amendment)
    2  
    3         Delete lines 545 - 563
    4  and insert:
    5  county.
    6         (a) The card must contain:
    7         1.(a) Voter’s registration number.
    8         2.(b) Date of registration.
    9         3.(c) Full name.
   10         4.(d) Party affiliation.
   11         5.(e) Date of birth.
   12         6.(f) Address of legal residence.
   13         7.(g) Precinct number.
   14         8.(h) Polling place address and a link to the supervisor’s
   15  website to provide the most current polling place locations.
   16         9.(i) Name of supervisor and contact information of
   17  supervisor.
   18         10.(j) Other information deemed necessary by the
   19  supervisor.
   20         (b) A voter information card may not be furnished to a
   21  voter unless the supervisor or the division has determined that
   22  the voter is eligible to vote.
