The Committee on Health Policy (Harrell) recommended the
following:
1 Senate Substitute for Amendment (560756) (with title
2 amendment)

4 Delete line 209
5 and insert:
6 parent’s his or her religious tenets or practices or conscience.
7 The exemption form must include an affirmation that the parent
8 claiming an exemption under this paragraph has consulted with a
9 physician licensed under chapter 458 or chapter 459 or an
10 advanced practice registered nurse licensed under chapter 464
11 about the benefits and risks associated with the vaccinations
12 required under subsection (3) before claiming the exemption. The
13 form must be signed by the physician or advanced practice
14 registered nurse to attest that the consultation was performed.
15 The department shall ensure that such consultations are
16 available, either in person or by electronic means, at no cost
17 at county health departments or other locations. The department
18 shall adopt rules to implement this paragraph’s consultation
19 requirement;

22 And the title is amended as follows:
23 Delete line 42
24 and insert:
25 such exemptions; requiring that the department ensure
26 certain consultations are available at no cost at
27 county health departments or other locations;
28 requiring the department to adopt rules; revising
29 requirements and procedures
