The Committee on Health Policy (Harrell) recommended the
following:
1 Senate Amendment (with title amendment)

3 Delete line 209
4 and insert:
5 parent’s his or her religious tenets or practices or conscience.
6 For a conscience-based exemption, the exemption form must
7 include a positive indication that the parent claiming such
8 exemption has consulted with a physician licensed under chapter
9 458 or chapter 459 or an advanced practice registered nurse
10 licensed under chapter 464 about the benefits and risks
11 associated with the vaccinations required under subsection (3)
12 before claiming the exemption. The form must be signed by the
13 physician or advanced practice registered nurse to attest that
14 the consultation was performed. The department shall ensure that
15 such consultations are available, either in person or by
16 electronic means, at no cost at county health departments or
17 other locations. The department shall adopt rules to implement
18 this paragraph’s consultation requirement for a conscience-based
19 exemption;

24 and insert:
25 such exemptions; requiring that the department ensure
26 certain consultations are available at no cost at
27 county health departments or other locations;
28 requiring the department to adopt rules; revising
29 requirements and procedures
