Electronic Health Records; Authorizes person to donate his or her electronic health records to specified entities for specified purposes; requires electronic health records donated by health care surrogate to be de-identified; authorizes donor to amend terms or revoke donation; requires AHCA & DHSMV to develop & implement program that encourages & authorizes persons to donate electronic records; revises suggested form for designation of health care surrogate.
