| Project Description: |
| Assistive care services include 24/7 on-site support. This is an all inclusive housing model serving a total of 41 individuals who are severely and persistently mentally ill. Services will include on site intensive case management, nutritionally balanced meals, transportation to doctors, life skill program, daily housekeeping and laundry, utilities, furnished rooms, medication supervision. |
