Local health planning councils (LHCs) provide professional, objective health planning services at the local, district and community levels. LHCs serve as non-profit agencies established by Florida Statute 408.033. LHCs represent local community interests by coordinating efforts to improve health status, promote access to care, reduce health disparities, and identify best practices in health care delivery. LHCs play an important role in translating data into decision-making and community tracking tools and are proposing to deliver this service through the development of community health report cards as a special project.

Project Description:
The Health Council of South Florida is requesting funding for the creation of community health report cards for the ll local health planning districts throughout the state of Florida. The purpose is to provide a valid and unified method of tracking health indicators over time to measure improvements in the community health. To date, these community-driven report cards have been shown to be an essential tool for improving community health and have been developed by more than 115 communities nationwide. To date, however, no statewide effort has been undertaken to employ this user-friendly tracking method. Florida is positioned to be a leader in this effort, working through the local health planning councils and a statewide advisory council involving the expertise of state universities and other health leaders to track 25-30 indicators on: Health Promotion and Wellness; Health Status; Access to Care; and Health Disparities as well as selected predictive indicators of community health. Flexibility will be employed to address local issues unique to particular communities.

Measurable Outcome Anticipated:
The measurable outcome of this project is to develop "best in class" Community Health Report Cards for the state of Florida. The process will include 25-30 selected health indicators for the 11 community health planning districts across the state of Florida to be utilized by state and local health officials, state legislators and other elected officials, private health foundations, and health care providers responsible for development of programs identified needs. The report cards will be available in hard copy and web-based format with links to detailed datasets for more rigorous and detailed analysis at the community level. Five-year trend analysis will also be presented for selected datasets.
