Statewide Interest:
A recent Needs Assessment in South Florida reveals the targeted population mainly the Haitians to be high risk for health outcomes. The population epidemiology (300,000 children and adults) has no health insurance in Miami-Dade County (LAU,2001). This number has increased from 2001 to 2004, there is a high percentage of Haitian women of childbearing age confronting ailments without health insurance, many of them attend delivery of their children with no prenatal care; some even consult a physician the day of delivery. The incidence and prevalence of high risk infants among such groups is the highest in the State of Florida 62% (Fetal Infant Mortality Rate) (FIMR, 2004). Haitians are also the highest for infectious diseases including HIV/AIDS, STIs, Tuberculosis, and other venereal diseases. This request is specially to reduce such discrepancies.

Project Description:
We want to reduce health disparities among cross cultural groups to include but not limited to African Americans, Haitians and other Caribbean (62%), needy Latino (23%) and Caucasians (15%). By providing primary health care to indigent patients residing in South Florida, the goals and objectives of this strategy planning is to increase access to primary health, thereby improving health status of the targeted population. The project will also reduce the incidence and prevalence of high risk infants, the rate of infectious diseases such and Human Immune Virus (HIV), the Acquired Immune Deficiency Syndrome (AIDS), Tuberculosis, Sexually Transmitted Infections (STIs), Hepatitis. The providers will also administer clinical care and health education for reducing the rate of morbidly and metabolic diseases such as Diabetes Mellitus type 2, Hypertension, high cholesterol and other related disorders.

Measurable Outcome Anticipated:
This plan will have a positive fiscal impact on the community because the beneficiaries are workers; they will be more productive at work since they will miss fewer days out sick, and women and children will be healthier. Male workers will also benefit from this primary care setting because they will produce more at work and student patients will have better grades.
