Project Description:
Currently, Jackson has 12 primary care centers, 7 school-based initiatives, 1 mobile health van as part of its primary care initiative. Given the need of the community, Jackson will expand its primary care efforts into underserved areas, all in effort to improve preventive care.

Measurable Outcome Anticipated:
By improving primary care efforts, patients will experience the benefit of preventive care and reduce the need for health care in an emergency toom or hospital inpatient setting, which is costlier for all involved.
