| Currently, across Florida the health and long-term care systems are separate and do not communicate for the benefit of the user. The Older Adult Planning project would improve coordination between resources for the elderly and provide a more effective use of funding for services. The project could serve as a model for the state on how to plan, design and implement new systems in each community for the future growth of older persons |

| Lee Memorial Health System is making a joint request with West Central Florida Area Agency on Aging and the Council on Aging of Bay County to plan, design and implement new systems, which will address the gaps and overlaps, and fragmentation in the long-term care systems, involving community based, in-home service providers along with those offering assisted living and nursing home care. |

| Results will include increasing numbers of well-trained geriatric care providers to provide quality services to older residents, broaden the array of prevention and wellness programs and information; increased utilization of behavioral health services by older residents and their caregivers; provide information to residents and caregivers about health and social serivices; develop and maintain appropriat and adequate service so older persons experience continuity of care from the health, social services and long-term care systems. |
