Palm Terrace Assisted Living Facility was a model project for the State of Florida, which was implemented in 2002. It serves as one of three model-project facilities in Florida that has received state funds throught the Department of Elder Affairs (DoEA). A goal of this model poject was to develop alternative affordable assisted living for frail, vulnerable elders who are "Section 8 Eligible" and at risk of nursing home placement. This project facility, as an alternative placement, is much more cost-efficient as compared to nursing home placement. The state has an interest in finding the most cost-efficient yet "needs-appropriate" placement for the growing population of elderly persons who cannot care for themselves in their own home. Such alternative placements will save the state a significant amount of money for an indefinite period of time. This model facility that was established by the DoEA will be able to replicated on a statewide basis in order to implement widespread state cost-savings as it continues to prove that it is a successful alternative placement for elderly persons who would otherwise be placed in expensive nursing homes paid by the state.

Project Description:
The Coming Home Project in Hillsborough County is structured so that the assisted living facility is able to provide care for elderly citizens by allowing the facility to utilize federal Section 8 rent vouchers, and additional supportive services funded through the Assisted Living Medicaid Waiver Program (such as personal care, medication management, etc.). The Coming Home Project is due to end effective June 30, 2005, if no other funding is received by the project. The intent for continuation is for these project facilities to be rolled into the Department of Elder Affairs (DoEA) Assisted Living Medicaid Waiver Program, administered by the Area Agencies on Aging. Unfortunately, as a result of Florida's Medicaid funding crisis, we cannot be certain that additional funding will be allocated to the DoEA Medicaid Waiver Assisted Living Program. A lack of funding will put these elderly people at risk of premature and more costly nursing home placement. As originally established, the facility was required to serve at least 37 low-income seniors under the Coming Home grant. As of November 2004, 43 elders were being served in the program and call Palm Terrace "home".

Measurable Outcome Anticipated:
A minimum of 37 frail & vulnerable elderly persons who are low-income would be served by living in Palm Terrace ALF through placement as an alternative to nursing homes. The Palm Terrace ALF would continue to be able utilize/leverage federal Section 8 rent voucher money to provide other other services which would allow elders to continue to live in the facility. A currently indeterminate but potentially large amount of money would be saved by the state by placing qualifying, low-income seniors who cannot live on their own in a less-costly alternative living arrangement compared to premature nursing home placement. With the project due to expire in June 2005, and in light of the Medicaid Reform activities, if this project is not funded again under the Medicaid Waiver Program, these elders would be placed in a more-expensive nursing home.
