Project Description: |
There has been an emerging public health crisis among African-Americans and Hispanic Americans--the Silent Epidemic of Alzheimer's Disease. Alzheimer's disease is more prevalent among African-Americans than among whites - with estimates ranging from 14% to almost 100% higher. Hispanics are twice as likely as non-Hispanic whites to develop Alzheimer's disease.
The Alzheimer's Association has targeted services to low income, minority and rural persons affected by this disease. In Miami-Dade County there are more than 300,000 persons over the age of 65 and, of those, more than 189,000 Hispanics over the age of 65 and 30,000 African Americans over the age of 65. These are compelling numbers with estimates of more than 52,000 afflicted with dementia. For each person with the disease at least three persons, usually family members, are also engaged in support and assistance.
This program, created in 2001, provides community education and outreach in the minority community.
Goals include:.
Increase and enhance community outreach, network, and awareness of dementia services to underserved minority individuals and families.
Increase awareness on disaster preparedness including hurricane response.
Increase and enhance support services, including a multi-language, 24/7 telephone support line.
Develop a culturally appropriate awareness campaign through faith-based and other community organizations that includes key leaders and members of the target communities.
Prevent costly crisis care often caused by a lack of preventive services.

Measurable Outcome Anticipated: |
Provide appropriate support services outreach, and awareness.
Continue working with faith communities, civic leadership and community organizations that serve target groups.
Develop and implement marketing/outreach strategy.
Increase the awareness of dementia services available to the target audience.
Increase the access of services available to the target audience.
Support Groups:
Continue culturally appropriate and specific support groups for African-American and Hispanic/Latino families.
Provide group facilitator training for church and community leaders; institute discussion opportunities for neighborhood groups.
Caregiver Training
Provide caregiving skills classes in the faith community setting and community centers as appropriate, and work collaboratively with Area Agencies on Aging and the provider network to reach specific populations. Caregiver curriculum to include Alzheimer's and dementia overview, diagnosis and treatment, communication skills, understanding behaviors, providing a safe environment, accessing community resources.
Care Consultations:
Assessment of needs, assistance with planning and problem solving (which may include the development of an action plan) and provision of support. Also may include: providing information and making appropriate referrals, supportive listing, and follow-up.
Community Collaborations:
Continue collaborative partnerships with faith-based programs and congregations; Sororities; Urban Leagues; appropriate Chambers of Commerce; the aging network; community health clinics; academia; media outlets; city and county government; leisure services; medical groups; neighborhood and civic organizations; and the philanthropic community.
