Project Description: |
The building will be a 31,661 square foot, one story facility that will be located on 6 acres of land in Brevard County and will initially care for 48-Special Care (Day Care), 24 Sundowners participants, Research Facility, Health Home Care, 4- 24 hour respite beds, 12-ALF (ECC) patients and 32-Skilled Nursing Beds (will accept overflow respite beds and (Hospice) patients in a non-institutional environment.
The core component is our own Multi-Therapeutic Intervention and Early Intervention Programs. First the care recipient first has complete assessment to evaluate the level of their functional ability and then is assigned a measurable indicator to establish their bas line. Through observations, the progress of each care recipient will be assessed and updated monthly against their initial baseline upon admission. This gives the research personnel and therapist an excellent tool for monitoring their client's progress with their psychological and physiological health. Our goal is to maintain the level of the original baseline, as long as possible, by adjusting the levels of the program to stabilize their functional capabilities. If we can treat the recipient in the early stages of this disease, we will have a better chance of extending a person's quality of life and maintaining the integrity of the family unit.
Sundowners, night care as well as day care will be available. (The syndrome known as "Sundowners" affects primarily people with Alzheimer's disease and when with families/or caregiver's see an increase in confusion and anxiety levels in their loved one's making the afternoon and evening hours more difficult for them to handle.
The approach of our Caregiver Outreach Program consists of establishing collaborative realtionships to facilate and provide outreach, referral, information, education and direct services through professionals and trained volunteers over the totality of the disease. We shall:
*Establish and Initial Evaluation Assessment for each caregivver to find out their needs.
*Individual and group counseling for the caregiver and recipeint.
*Home Health Care
*Provide an Alzheimer's and dementia resource and Service Guide for each caregiver.
*Establish and educational program that will be offered during the stages of Alzheimer's disease (Early Stage, Middle Stage, Late Stage).
*Increase awareness in Medical Updates; Finances and Law concerns; Home Safety Issues and Day-to-Day Skills through the total progression of the disease.
*Develop an Alzheimer's and demential Stress Management network.
Addressing our last component of The Embers Model, 24-hour Respite Services. Caregivers who need a break from care giving will be able to take advantage of one day, one week, two weeks or even a month if necessary.Alzheimer's patients will be provided a private room with attending nursing staff. We have 4 Respite beds for this purpose. Each day they will participate in the Multi-Therapeutic Intervention program to maintain continuity while residing at the Center. The Embers will work closely with the East Central Florida Memory Clinic (ECFMC), one of the thirteen Memory Disorder Clinics funded through the State of Florida's Department of Elder Affairs Alsheimer's Dieseeas Initiative to provide free mimory screenings, caregiver and professional dementia education, and active involvement in research. By partnering, The Embers and ECFMC, both non-profit agencies, will be utilizing resources and services in the area in a cost-effective manner to directly impact the person with Alzheimer's disease and the caregiver. In addition, the Embers ability to serve the community is enriched by the Momory Clinic's relationships with Florida Institute of Technonlgy and Health First. Each of these affiliations, a university and a helathcare system, allow for enhanced research opportunities. Our in-house professional speech and physical therapist staff will avail themselves with the latest research in developing new therapies. They will also train our CNA's as Restorative aides to perform maintenance level therapies, established by the therapist. We have improved walking, feeding and other required functions to maintain ones quality of life thus preventing early nursing home placement. This facility will house the hub for all the components of this model including our own Health Home Care Aides to assist caregivers at home. With our nursing capabliity we will be able to treat minor medical problems that could place the care recipient in early nursing home placement or require the caregiver to stay at home from work to care for their loved one.
A little known fact is that to date there are over 11,601 diagnosed cases of Alzheimer's disease in Brevard County alone. Initial marketing surveys indicate that The Embers will have a 6.5% penetration rate in all Brevard County, with this penetration rate, it is estimated that we will capture 56% of that rate in Cocoa/Rockledge area.
The outcomes are to maintain their quality of life, as well as maintain their dignity while encouraging the integrity of the family unit. These Centers are able to support any individual community and will allow families to stay together longer. This will help reduce the emotional and financial burden placed on the caregiver.
There is an urgent need for this type of facility. There are many agencies who provide counseling and some type of intervention to an Alzheimer's sufferer and his/her family, but as yet, such an all encompassing facility as ours is not available in this area. With our new pioneering Embers Model, we will have a new and cost effective approach to the continuous care of our Alzheimer's and dementia population in Brevard County, the State of Florida, and throughout The United States.
This new humanistic and cost savings model for the care of Alzheimer's and other related dementia's is needed now. Remember the cost of Day Care is less than half that of Nursing Homes |

Measurable Outcome Anticipated: |
With the implementation of our programs for caregiver outreach and our multi-therapeutic intervention, we hope to solve these challenges, reflected in (7). This in turn will lengthen time spent in home care and reduce the stress factor and depression with caregivers, to stop early placement into long-term care by maintaining the family unity.
Because of excessive stress and lack of proper support the participant is placed in a nursing home to enable the caregiver to escape. We are designing our program to eliminate or reduce this practice, which has placed excessive financial burden on the Medicaid system. With preliminary data projected over a ten-year period, using 2004 cost, we estimate we could see overall savings of about 40 to 48% in overall health care cost over a ten-year period depending on market conditions. |
