Project Description:
Gateway Community Services, in partnership with Sulzbacher Center for the Homeless & Mental Health Resource Center, Inc. plans to provide innovative, integrated treatment to persons who are homeless & have a co-occuring substance use & mental health disorder. Originating in Jacksonville, Florida, this program will include psychiatric evaluation & counseling, substance abuse treatment, intensive case management, medication management, housing referral & crisis management.

Measurable Outcome Anticipated:
60% of participants, who remain in the program, will have reduced alcohol & illegal drug activity within 6 months of enrolling into program. 60% of participants who remain in the program will reside in permanent housing 12 months from enrollment into the STAR Program & will not revert back into homelessness. 70% of all STAR participants who remain in the program will be employed or receiving government assistance to ensure financial & residential stability 12 months after enrollment into the STAR Program. 60% of participants who remain in the program will have no or a reduced incidence of involvement with the criminal justice system in a 12 month period.
