Project Description:
Hope of Southwest Florida is requesting funding for 200 additional slots for the new hospice based PACE program in Southwest Florida that was approved by the 2004 Legislature to open in 2005. The initial 50 slots granted by the Legislature at that time, plus the 200 asked for in this request, would provide a total of 250 slots when the program opens as scheduled in the Fall of 2005. The PACE program allows seniors of our state with chronic care needs to be served in the community when possible. For an all-inclusive capitation rate, the medical cost reduction program provides eligible seniors with adult day care centers offering nursing, therapies, meals, nutrition, social work and personal care. Services offered include: primary medical care, specialty medical care, hospitalization, nursing home care, prescription drugs, home health care, social services, transportation, and respite care.

Measurable Outcome Anticipated:
Makes best use of Medicaid/Medicare dollars allowing enhancement with private funding; Service package emphasizes preventative care that is usually less expensive and more effective than acute care; Documented increased life expectancy, fewer hospital and nursing home days; Savings to Florida Medicaid will be realized by replacing nursing home placement costs with the lower capitated PACE Medicaid rate.
