Project Description: |
TNNI Precision Care is seeking financial assistance from the State of Florida to provide home care services to the frail ederly individuals that are physically and mentally challenged and underserved/uninsured in Miami-Dade and Broward Counties. The funding will be used to expand TGNI Precision Care's services to these selected counties, which have been identified as having difficulties of their Activities of Daily Living (ADL's) and their Instrumental Activities of Daily Living (IADL's). Based on the Department of Elders Affairs (DOEA) report on the Miami-Dade County Level Data, it indicates that the population of elders that are 60+ are more that 414,608. More that 86,923 (20.9%) in poverty, 94,148 (22.7%) are low income <125% of poverty level, 30,661 (7.4%) are with Mobility Self-care limitation, 81,806 (19.7%) that are living alone and 45,520 (10.1%) probably suffer from Alzheimer's disease, and only 37,530 that are enrolled in all the DOEA Programs combined. |

Measurable Outcome Anticipated: |
With the funding requested, TGNI Precision Care will be able to provide the best quality continous home care services to enhance the lives of 60 frail elderly individuals that are physically and mentally challenged and to enable them to be aged in place with dignity and with a purpose; recogniaing the cultural diversity of the community that we serve, it is estimated that 4 to 6 hours/units of services will be delivered a week based on client's needs, and they are as follow: Screening/Assessment; Companion; Escort; Homemaker; Personal Care; Shopping Assistance; Telephone Reassurance; In-Home Respite and Interpreter/Translations. |
