Project Description: |
The Behavior Modification Outcome Program is a uniquely developed program that identifies young, pregnant women, who have a history of incarceration get a new lifestyle and begin to change the future, for their babies and themselves. This program provides prevention, inervention, and after-care services taht address some of the root causes of the floating of the juvenile and criminal justice systems. The objective of the Behavior Modification Outcome Program is to provide young expectant mothers positive socialization skills, jobs adn educational opportunities and Lifestyle changes. Services to be provided are Educational, Employability and Behavior and Skills, job referral & placements, psychological, medical healthcare and multicultural services. |

Measurable Outcome Anticipated: |
100% of all mothers and babies will receive medical healthcare; 100% of all mothers will receive Psychological Services. At least 60% of all needy clients shall obtain employment within 120 days of intake. At least 70% of identified clients shall demonstrate improvement in skill levels on completion of prescribed education or vocational programming. At least 75% of clients participating in the job readiness training shall complete the training. All clients requiring housing will receive assistance in establishing a housing plan and a referral to a social service agency. Follow-up will occur on all clients for 30, 60 and 90 days after placement in jobs, school or other services. 100% mothers and babies will have a life style service treatment plan developed. |
