| Project Description: |
| Currently, Jackson has several disease management programs, which have significantly increased over the past several years. We target chronically ill patients in areas such as asthma, diabetes, congestive heart failure, HIV/AIDS, high risk pregnancies and othe diseases. By educating patients on their disease and teaching them to manage and recognize thier symptoms, we have saved significant dollar amounts by reducing their hospital and emergency room expenditures. |
