| Project Description: |
| TGNI Precision Care is seeking financial assistance from the State of Florida to provide home care services to the frail elderly individuals that are physically and mentally challenged and underserved/uninsured in Miami-Dade County. The funding will be used to expand TGNI Precision Care's services to the underprivileged area in the County, and to identify and serve elders having difficulties with their Activities of Daily Living (ADL's) and their Instrumental Activities of Daily Living (IADL's). The Department of Elders Affairs (DOEA) report on the Miami-Dade County Level Data indicates that the population of elders who are 60+ are more than 414,608. More than 86,923 (20%) live in poverty, 94,148 (22.7%) are low income with <125% of poverty level, 30,661 (7.4%) have Mobility Self-care limitations, 81,806 (19.7%) are living alone and 45,520 (10.1%) probably suffer from Alzheimer's disease. Only 37,530 are enrolled in all the DOEA programs combined. |

| Measurable Outcome Anticipated: |
| With the funding requested, TGNI Precision Care will be able to provide the best quality of continous home care services to enhance the lives of an additional 20 frail elderly individuals who are physically and mentally challenged and to enable them to age in place with dignity and with a purpose while recognizing the cultural diversity of the community that is served. It is estimated that 4 to 6 hours/units of services will be delivered a week based on client needs. These needs are as follows: Screening/Assessment; Companion; Escort; Homemaker; Personal Care; Shopping Assistance; Telephone Reassurance; In-Home Respite and Interpreter/Translations. |
