| Healthcare - reduction of emergency room visits in West Pasco County will reduce the state's Medicaid budget and 3rd party payments that are being paid for non-emergent, non-trauma cases in the emergency room. The patients will be able to come to the Premier's Community Health Center for a medical home that will in turn make available primary and preventative care services that will be the catalyst for a healthier West Pasco County. It will serve as a model for all of Florida's community health centers. |

| Project Description: |
| Providing a medical home for West Pasco residents to have access to primary and preventive healthcare. In 2005, Premier Community HealthCare Group, a federally qualified community health center (FQHC) and migrant health center (MHC) treated 5,604 patients who were either un/underinsured. The goal is that each and every resident have a medcal home to receive their care instead of the emergency room that costs 10 times more than it does to recieve that care in a community health center. We know that community health centers are the model for tomorrow and provide the much needed continutity of care that will be the catalyst for a healthier Pasco County. We believe that "Health is Wealth" and by keeping individuals healthy they can work and sustain the continued economic stability and development of Pasco County. |
