Health Insurance/Rate Filing; modifies rate filing requirements for approval of health insurance policy forms by Insurance Dept.; provides guidelines for determining when benefits are considered reasonable re premium charged for purposes of disapproval of health insurance policy forms by department. Amends 627.410,.411.
