Health Care; creates Mandated Health Insurance Benefits & Providers Estimating Conference; prohibits billing HMO subscriber for services covered by HMO; amends membership of statewide provider & subscriber assistance panel; requires physicians & hospitals to post sign & provide statement informing patients about toll-free health care hotline; provides HMO subscriber's bill of benefits, etc. Amends Chs. 216, 395, 400, 455, 408, 624, 641.
