Small Employer Health Benefit Plans; revises definition; requires small employer carriers to begin to offer & issue all small employer benefit plans on specified date; deletes requirement that basic & standard small employer health benefit plans be issued; provides additional requirements for determining premium rates for benefit plans; provides for application to plans provided by certain small employer carriers under certain circumstances. Amends 627.6699.
