Continuity of Care for Medically Stable Patients; Prohibits insurance policies, HMO contracts, & pharmacy benefit managers from limiting or excluding coverage for, increasing payments for, or adjusting tiered formulary with respect to certain drugs approved for coverage of specified medical conditions; authorizes generic substitutions; applies prohibition to group health insurance, blanket health insurance, & franchise health insurance.
