Health Insurance Cost Sharing; Defining the term “cost-sharing requirement”; requiring specified individual health insurers and their pharmacy benefits managers to apply payments by or on behalf of insureds toward the total contributions of the insureds’ cost-sharing requirements; requiring that contracts require pharmacy benefits managers to apply payments by or on behalf of insureds toward the insureds’ total contributions to cost-sharing requirements; providing contribution to cost-sharing requirements for small employer carriers, etc.
