Project Description:
Central Florida Health Care, Inc. (CFHC) is one of 27 private, not for profit 501c3 corporations spanning across Florida and one of 1300+ health centers across the country. CFHC's Centers are located within the Counties of Polk (since 1972), Hardee (since 1975), and Highlands (since 1980). Our centers are one of Florida's oldest primary care "safety net" providers and one of the "original" gatekeeps. Since 1972, CFHC has provided a medical and dental home to thousands of patients. CFHC is a community based health care corporation, governed by a volunteer community-based board of, by and for the people to ensure that affordable, tailored health care services are provided within our underserved communities. Central Florida Health Care (CFHC) aims to bridge the gap between the health outcome currently experienced by the uninsured and underinsured population in Hardee County. Central to thier mission is provide affordable, comprehensive healthcare to thier patients. Since CFHC is keenly aware of the external environment changes across the County and across the State and we form alliances that are destined to strengthen our centers. Our modest budget and mission to serve patients based on thier ability to pay places us at a significant disadvantage. CFHC seeks state funds to enhance CFHC's dental program. These funds will be used to improve existing dental operations. Increasing access to oral health services will result in better health outcomes with this County and a lessening of the unwarranted health disparities. Health status outcomes will flow from increased expansion of services and increased accessibility. CFHC's dental program will offer: emergency, diagnostic, preventve and basic restorative, prosthetics (in house lab) services and education.

Measurable Outcome Anticipated:
Increased physical access - thus removing an important obstacle to oral health care access. Enhanced patient privacy. Increase the pace with which patients will be provided with services, thus reducing time away from work, school, family or other required obligations. Preventative and primary oral health services to greater numbers of uninsured and underinsured will reduce the amount and duration of illness, result in earlier diagnosis of afflicitions, enable consumers to receive earlier treatment for dental care. CFHC has demonstrated performance record in managing multi-site health delivery system. In sum, this proposal will signifcantly increase the use of preventive health service, and ensure more effective care which can be measured.
