Child Care Facilities; requires signed affidavit attesting to accuracy of certain info. provided by applicant for child care facility license; requires CFS Dept. to establish database of info. re violations, citations, & penalties imposed against child care facilities regulated by state; requires said department to consult & meet requirements of State Technology Office; provides for transfer of Child Care Program from CFS Dept. to Health Dept., etc. Amends Ch. 402.
