Individual Education Plans; Provides school district requirements relating to determination & IEP meetings; requires school district to take specified actions if related service identified in student's IEP is not provided; provides that parent or guardian has right to request provider logs or notes within specified timeframe; requires school district to inform parents of such right; requires each school district to provide individualized orientation to each parent or guardian of student newly identified to be eligible for exceptional student education services; provides requirements for orientation; requires school district to obtain signed acknowledgement from each parent or guardian; requires that annual refresher orientations be offered & documented; & requires that certain information be retained in student's education records.
