494.0066 Branch offices.--
(1) A permit is required for each branch office maintained by a licensee under ss. 494.006-494.0077.
(2) The department shall issue a permit upon receipt of a completed application form as prescribed by the department and an initial permit fee set by rule of the department in an amount that may not exceed $250. The branch office application must include the name and license number of the licensee under ss. 494.006-494.0077, the name of the licensee's employee in charge of the branch office, and the address of the branch office. The permit shall be issued in the name of the licensee under ss. 494.006-494.0077 and must be renewed in conjunction with the license renewal.
